Duplicate request forms present a particular difficulty for the laboratory. Patients may present with request forms from a hospital clinic or private specialist and a general practitioner. It may appear a simple step to combine the requests onto one form and to copy the results to the other practitioner but there are difficulties if the test requests do not match exactly. Issues arise such as who becomes the primary requestor and takes responsibility for the results.
Unnecessary duplication can be avoided by asking patients about any current request forms they hold, particularly serial request cards, before issuing a new laboratory request.
